You Don't Have to Look Like the Stereotype to Have OCD
Most people picture OCD as excessive hand-washing or checking locks. But for high-achieving adults, OCD often looks completely different and far more invisible.
It might look like this:
Spending hours mentally replaying a conversation, searching for proof you didn't say something wrong
Needing certainty before making a decision — and never quite feeling certain enough
Intrusive thoughts that horrify you, followed by intense guilt or shame for having them
Repeating rituals — mental or physical — to neutralize anxiety, even when you know they don't really help
Avoiding situations, relationships, or responsibilities because the anxiety isn't worth it
If any of that sounds familiar, you may have OCD — and you've likely been managing it alone for a long time.
What Is OCD, Really?
OCD (Obsessive-Compulsive Disorder) is a cycle. An intrusive thought, image, or doubt triggers intense anxiety or distress (the obsession). You respond with a behavior — a compulsion — designed to reduce that distress. The relief is temporary. The cycle repeats, and over time it grows.
OCD is not a personality quirk or a sign that something is deeply wrong with you. It's a well-researched, highly treatable condition — and it's far more common in high-functioning, intelligent adults than most people realize.
Common OCD subtypes in high-achieving adults include:
Harm OCD
intrusive fears of hurting yourself or others
Relationship OCD (ROCD)
relentless doubt about your partner, your love, or whether your relationship is "right"
Pure O
primarily mental obsessions without obvious external compulsions
Scrupulosity
moral or religious obsessions and guilt
Health Anxiety OCD
constant fear of illness despite reassurance
Perfectionism OCD (“Just Right” OCD)
the need to do things "just right" or redo them until they feel correct
Food Allergy OCD
When legitimate allergy awareness crosses into OCD territory — repeated checking, reassurance-seeking, and anxiety that no amount of preparation ever fully quiets. ERP works within the reality of your medical situation to identify where reasonable precaution ends and OCD begins.
Emetophobia (Fear of Vomiting)
An intense, persistent fear of vomiting that drives restrictive eating, avoidance of social situations, and a life increasingly organized around one fear. More common in high-achieving adults than most people realize.
What Is ERP and Why Does It Work?
Exposure and Response Prevention (ERP) is the gold-standard, evidence-based treatment for OCD. It's the approach with the strongest research support — and it's fundamentally different from general talk therapy.
Here's the core idea: OCD is maintained by avoidance. Every time you perform a compulsion to escape anxiety, you teach your brain that the thought was dangerous and that relief requires that behavior. ERP works by gradually and deliberately breaking that cycle.
In ERP, we:
Identify your specific obsessions, compulsions, and avoidance patterns
Build a hierarchy of feared situations, from least to most distressing
Practice sitting with uncertainty and anxiety — without performing compulsions
Help your brain learn, through direct experience, that the thoughts are not dangerous and that you can tolerate discomfort without catastrophe
ERP is not about eliminating intrusive thoughts. It's about changing your relationship to them — so they lose their power to run your life.
OCD Therapy at Nix Baker Wellness
I'm Brittany Baker, LCSW-S, and I specialize in ERP for adults who've spent years white-knuckling their way through OCD without ever knowing that's what it was. Maybe you have been told you have anxiety or ADHD, but really you have been struggling with OCD.
My clients are typically high-functioning — They are high-level thinkers. They've built successful careers, maintained relationships, and kept most people around them from noticing the internal weight they carry. What brings them to therapy is the exhaustion of managing something that never fully goes away on its own.
My approach to ERP is structured, honest, and deeply collaborative. I'll explain every step of the process, we'll work at a pace that challenges you without overwhelming you, and I'll never ask you to do something I haven't thought carefully through with you first.
OCD therapy is hard work. It's also some of the most meaningful and lasting change I get to witness in my clients.
OCD Therapy FAQs
Does ERP mean I have to face my worst fears right away?
1
No. ERP is graduated — we start with challenging but manageable situations and build from there. You're never thrown in the deep end. The work will be difficult though and challenge you every step of the way
What if my OCD is "just" in my head — no visible rituals?
2
Mental compulsions are still compulsions, and actually VERY common. ERP is still the right treatment. "Pure O" is one of the most commonly misunderstood and undertreated forms of OCD, and it responds well to ERP.
Can I have OCD and anxiety or ADHD at the same time?
3
Yes — and it's very common. Many of my clients present with overlapping OCD, anxiety, and ADHD. Part of our work together is understanding what's driving what, so we can treat the right thing in the right way.
How long does ERP take?
4
This varies, but many people see meaningful progress within 12–20 sessions. ERP can extend past 20 sessions depending on your pace and needs. The treatment plan is individualized to your needs. OCD is highly treatable, and ERP produces results faster than most other approaches.
Do you offer telehealth OCD therapy?
5
Yes. I provide OCD therapy and ERP treatment online for adults in Colorado, Texas, Illinois, and Arkansas, as well as in-person in Denver, CO.
You've Been Managing This Long Enough
OCD doesn't get better with more willpower, more reassurance, or more avoidance. But it does get better with the right treatment — and the right therapist.
